PROCEDURE FOR INCIDENT AND INJURY REPORT
When a worker has an incident or injury, they need to report this to the supervisor/office orally as soon as possible, and then complete the Incident and Injury report form.
Below are steps to be taken by office worker (For office use only):
STEPS DETAILS CHECK
- Check that all the necessary information has been filled in on Incident and Injury report form by the worker.
- Check the severity of the situation and consult with Supervisor/Manager ASAP.
- Flag Manager if Incident and Injury report is significant.
- If it is a Worker Compensation Claim, Inform Manager, to provide Accounts Manager with the original document.
- Manager to act on the Incident and Injury report form ASAP by contacting worker and discussing nature, severity and steps they took to lessen the risk and possible long term solution.
- Accounts Manager to be advised if the worker needs to see a doctor, an Insurer Workers Compensation Claim form need to be completed by the worker who should provide a WorkCover Medical Certificate.
- The Workers Compensation Claim needs to be lodged within 5 days from the date of injury. If the worker is totally unfit and not able to provide the information within 5 days, the Insurer should be contacted and advised of the situation.
- Data entry on Podio.
- An accident investigation should be carried out and control measure to be implemented. i.e. review of procedures by Managers.
- Once prevention measures are implemented - record completion.
- Once controls have been taken the Observer should inspect the resolution and if agreed that the Incident and Injury report form has been resolved sign off on the Incident and Injury report form.
At each Safety Meetings, Incident and Injury Reports are discussed with workers and action or control measure taken. Also a review to ensure that the controls are still working and the best outcome is achieved.
Unresolved Incident and Injury forms to be brought forward and once resolved, then filed in the appropriate section of the Folder. Result to be taken and discussed in the next Toolbox/Safety meeting.
Incident and Injury report forms should be archived and kept for years.
